The forms look good. A required use of these forms would clarify my investment in mutual funds.

I recommend you require these to be used for new funds or when a new class is being considered by the investor.

If an investor is making regular investments say monthly, or even annually in the same fund, you might nbot require this form to be used for each investment.

However, if the class is to change or if the investor is investing in a differenht fund, then the form should be required.
