On July 15, 2009, Biolase Technology, Inc. (the "Company") reported on a Current Report on Form 8-K (the "July 15, 2009 Form 8-K") that, effective July 14, 2009, Frederick M. Capallo resigned from his position with the Company. This amendment is to clarify a typographical error made in the July 15, 2009 Form 8-K. Mr. Capallo resigned as the Company's interim Chief Financial Officer.
