Information disclosed under this form is to be used for purposes of underwriting my application in order to determine eligibility for life insurance, long-term care insurance if the Long-Term Care Rider is elected, and to detect fraud or abuse. This medical or health information may include information on the diagnosis and treatment of mental illness; alcohol; and drug use. This also may include information on the diagnosis; treatment; and testing results related to HIV; AIDS; and sexually transmitted diseases, unless otherwise restricted by state law.
