NOTE 15 - EMPLOYEE BENEFIT PLANS

The Company offers a 401(k) plan which is eligible to substantially all employees in the United States. For the years ended December 31, 2015 and 2014 as well as the first quarter of 2016, the Company elected to match employee contributions at a rate of 50 percent of up to 4 percent of salary deferral. The Company contributed $0.4 million, $2.6 million, and $2.5 million to the 401(k) plan in the years ended December 31, 2016, 2015, and 2014, respectively.  In 2016 the Company suspended indefinitely the employee match program.  The Company contributed in the first quarter of 2016 $0.4 million to the 401(k).  No other contributions were made during the remainder of 2016.
